ALBERT CHANG, MD

Dr. Chang is a Chicagoland native who completed his Physical Medicine and Rehabilitation residency at the University of Washington and is nearing completion of his subspecialty fellowship training in Pain Medicine at Virginia Mason Medical Center. He received his Bachelor of Science degree in Biology/Psychology with highest honor from the University of Illinois at Urbana-Champaign followed by his Doctor of Medicine degree at the University of Virginia. 

Dr. Chang’s approach to patient care is grounded in a pragmatic philosophy that prioritizes maximizing patient function and enhancing their quality of life. He strives to listen closely to patients, understand their unique goals, and collaborate with them to develop tailored treatment plans. Dr. Chang’s extensive training has equipped him with advanced skills in both fluoroscopic-guided and ultrasound-guided interventional spine and joint procedures, as well as electrodiagnostic medicine. Whether it’s alleviating discomfort, restoring mobility, or providing educational counsel on navigating the complex path to recovery from motor vehicle related injuries, Dr. Chang is committed to delivering compassionate and evidence-based care that empowers his patients to return to the activities that give them joy and purpose.

As a Midwesterner still settling into the Pacific Northwest, outside of work Dr. Chang continues to explore local traditions and activities. You might catch him dinking at the local Pickleball courts, begrudgingly camping in the North Cascades, or attempting to appreciate various artisanal coffees

david castle, mba, PA-C

David Castle is a board-certified Physician Assistant with over 20 years of experience in orthopedic surgery, trauma care, and injury medicine. He has a strong background in treating acute and chronic musculoskeletal injuries and has served in both clinical and leadership roles throughout his career.

Before joining Coast Injury Medicine, David was the Associate Medical Director of Orthopedics at Washington Permanente Medical Group, where he led a high-volume orthopedic practice and supported a team of surgeons and advanced practice providers. He has advanced training in fracture care, joint injections, musculoskeletal ultrasound, and surgical first assist.

David began his medical career as a U.S. Army flight medic and instructor, where he provided critical care in high-stress environments. He holds an MBA in Healthcare Management and earned his PA degree from the University of Washington’s MEDEX Northwest program.

David is committed to helping patients recover quickly and safely so they can return to the activities they enjoy.

Outside of work, David enjoys fishing, gardening, traveling, and supporting local high school sports. Earlier in life, he worked as a professional fishing guide and winter caretaker in remote Southeast Alaska—an experience that deepened his appreciation for the outdoors and self-reliance. He now lives in Snoqualmie with his family and enjoys spending time in nature across the Pacific Northwest.

IRISH DORSEY, PA-C

Irish Dawn A. Dorsey, PA-C holds an active Washington State Physician Assistant license, certified by the National Commission on Certification of Physician Assistant (NCCPA). She graduated from Morehead State University in Kentucky with a Bachelor of Science in Biology and Chemistry. She completed her Master of Science in Physician Assistant Studies (MSPAS) degree at South University in Tampa, Florida. Miss Dorsey has over 10 plus years of experience practicing as a physician assistant. 
 
As a result of her years and vast array of experience in orthopedics, regenerative medicine and pain management, she is skilled in many areas of patient care and treatment. As a lover of the great outdoors, she is dedicated to seeing her patients improve their quality of life and get back to the activities they love the most. 

Outside of work, Irish enjoys cooking/baking, hiking, rock climbing, traveling and spending time with her friends and family preferably sharing a meal together.

JAMES YING, MD

Dr. Ying is a board-certified physiatrist with fellowship training in interventional spine and musculoskeletal medicine. His clinical practice focuses on diagnosis and treatment of motor vehicle collision and work-related injuries.

Dr. Ying has extensive training in electrodiagnostic, interventional, and regenerative medicine for the treatment of spine and musculoskeletal disorders. His expertise includes fluoroscopic and ultrasound guided procedures, epidural steroid injection, facet joint injection, radiofrequency ablation, platelet-rich plasma (PRP), intramuscular stimulation (trigger point needling), and vertebral augmentation. He utilizes a combination of patient history, physical examination, electrodiagnostic studies (EMG/NCS), advanced imaging, and other studies as needed to reach the most accurate diagnosis.

Dr. Ying graduated with honors from Johns Hopkins University with a degree in biomedical engineering. He completed medical school at the University of Texas Southwestern Medical Center. After internship at Baylor College of Medicine, he completed his residency in physical medicine and rehabilitation (physiatry) at the University of Washington. After residency, he completed further fellowship training in interventional spine at the Swedish Neuroscience Institute. Having lived and trained on the East Coast, the South, and now the Pacific Northwest, Dr. Ying is comfortable interacting with patients from diverse backgrounds and strives to tailor his treatments to their individual needs.

As a physiatrist, Dr. Ying uses an interdisciplinary approach to his patient care with the goal of improving patients’ function and quality of life. Dr. Ying’s philosophy revolves around providing personalized comprehensive care to his patients using the most conservative and least invasive treatment methods. This involves a patient-centered approach to work with a team of physicians, surgeons, physical and occupational therapists, chiropractors, massage therapists, acupuncturists, mental health providers, and case managers.

Outside of work, Dr. Ying enjoys spending time with his family and pursuing a variety of hobbies including hiking, tennis, soccer, traveling, video gaming, and exploring the Pacific Northwest.

Jordan powell, md

Dr. Powell is a board-certified Physiatrist and Pain Medicine physician dedicated to restoring function and improving quality of life for patients with musculoskeletal injuries, spine conditions, nerve disorders, polytrauma, and complex pain syndromes.

His approach emphasizes evidence-based, individualized care, aligning treatment plans with each patient’s goals and values to help them return to work, recreation, and daily activities.

Dr. Powell earned his M.D. from Saint Louis University and served a decade in the U.S. Army. He completed his PM&R residency at Walter Reed National Military Medical Center and later became Chief of PM&R at Brooke Army Medical Center. At the Center for the Intrepid, he treated severe battlefield injuries and special operations forces, helping patients regain independence. He also co-founded a PM&R clinic for musculoskeletal and sports injuries.

After fellowship training in Pain Medicine, Dr. Powell led the Interdisciplinary Pain Management Center at William Beaumont Army Medical Center, directing a multidisciplinary team delivering comprehensive pain and rehabilitation care.

Dr. Powell holds certifications in PM&R, Pain Medicine, and Electrodiagnostic Medicine. He specializes in fluoroscopic spine procedures, regenerative medicine, joint and tendon interventions, and electrodiagnostics. As an RMSK certificant, he has advanced expertise in musculoskeletal ultrasound and is a Certified Strength and Conditioning Specialist (CSCS), reflecting his commitment to exercise-based recovery.

He has held academic appointments at the Uniformed Services University and UT Health San Antonio, publishing research on limb loss, ultrasound, and pain medicine.

Outside of medicine, Dr. Powell enjoys life with his wife and daughter. A former NCAA wrestler and national weightlifting competitor, he now spends his time reading, exercising, and caring for cats.

Joshua Grant, PA-C, MCHS

Joshua Grant is a board-certified Physician Associate bringing over 13 years of specialized experience in orthopedics and spine care. A graduate of the University of Washington MEDEX PA program, Josh devoted much of his early career to the complex surgical and non-surgical management of spine conditions, while also developing broad orthopedic expertise across outpatient and urgent care settings.

Throughout his career, Josh has served on the faculty with Johnson & Johnson, lecturing on perioperative wound closure and pain management. He has held clinical leadership roles and pursued entrepreneurial ventures in medical device and clinical care. Now settled in the Olympia area, Josh is excited to bring that depth of experience to his patients, helping them find meaningful relief from their injuries.

Josh believes that informed, engaged patients achieve better outcomes, and that philosophy is at the heart of how he practices. He is committed to educating his patients and working alongside them to build comprehensive, personalized treatment plans.

Outside the clinic, Josh embraces everything the Pacific Northwest has to offer. He and his wife enjoy adventures with their three kids, whether that means hitting the road in their RV, cheering on the Rainiers or Mariners, or exploring a new local restaurant. A self-described coffee enthusiast, Josh has a passion for specialty coffee and espresso, and even roasts his own beans at home.

JULIA JOHN, PA-C

Julia John is a board-certified Physician Assistant and proud Texas native with clinical experience in Interventional Pain Management. She earned her Bachelors of Science in Biology from the University of Texas at Dallas,  followed by her Physician Assistant degree from UT Southwestern Medical School in Dallas. 

After graduating, Julia immediately began her career in Interventional Pain Management and has been passionate about the field ever since. She has worked closely with patients dealing with a wide range of chronic pain conditions and has assessed them to determine the need for physical therapy, minimally invasive interventions, neurosurgical consultation or spinal cord stimulation. 

Julia is passionate about working in pain management and finds purpose in helping patients regain function, improve their quality of life and feel heard and supported. She’s drawn to creating individualized conservative care plans that prioritize long – term wellness. 

Outside the clinic, Julia enjoys spending time with her husband, traveling the world, and staying active whether it’s through working out, hiking or playing pickleball. She brings the same energy, curiosity, and dedication to her personal life as she does to every patient encounter.

Michael siem, pa-c

Mike is a dedicated Physician Assistant with over 16 years of experience in diverse medical fields. His journey began in the U.S. Army, where he served as a medic and later as a physical therapy assistant. After his honorable discharge, he pursued his passion for medicine through the University of Washington’s MEDEX program, earning his Physician Assistant degree.

Initially specializing in orthopedics for over eight years, Mike transitioned into interventional radiology, further expanding his expertise. Most recently, he has focused on addiction medicine, combining his clinical skills with a compassionate approach to patient care.

Mike is passionate about leveraging his extensive experience to help patients achieve full functional recovery, enabling them to return to work and enjoy life to the fullest.

Outside of his professional life, Mike enjoys spending time with his wife and teenage son. An avid cook, he has a particular love for barbecue. He also enjoys reading, photography, and music. Having lived in 13 states, Mike now calls Washington home, having resided here longer than anywhere else. He is also an avid traveler, eager to explore more of the world in the future.

mikaela serafin, lmhc

I am a Licensed Mental Health Counselor (LMHC) and Certified Trauma Specialist with more than five years of clinical experience. I hold a dual master’s degree from The Seattle School of Theology & Psychology, as well as two undergraduate degrees from Cal Poly San Luis Obispo. My education and experience have shaped an approach to care that combines clinical knowledge with warmth, compassion, and a deep respect for each person’s individual story.

 

I work with individuals experiencing anxiety, depression, PTSD, and the emotional effects of trauma. I specialize in Lifespan Integration, a gentle, neuroscience-informed therapy that helps the mind and body process trauma at its roots and develop a greater sense of safety and wholeness.
As someone who has personally experienced a serious car accident, I understand how chronic pain and injury can affect every part of a person’s life. I also understand how difficult it can be to adjust when life suddenly looks very different from what it did before. This experience has strengthened my belief that healing is not only about addressing individual symptoms—it is also about rebuilding connection, restoring a sense of safety, and creating space for growth and hope. I am passionate about creating an environment where people feel seen, supported, and empowered as they move toward healing.
Outside of the office, I enjoy strong coffee, supporting local farmers markets, and cheering for the San Francisco Giants—although I am developing a soft spot for the Mariners. On any given day, you will most likely find me at a favorite local coffee shop with a good book, my partner, and our dog, Rainey.

Neil Batta, MD

Originally hailing from Buffalo, Dr. Neil Batta is a dedicated interventional pain physician with a background in Physical Medicine and Rehabilitation (PM&R). After earning his undergraduate degree from St. Bonaventure University, he obtained a doctor of medicine (M.D.) at The George Washington University School of Medicine and Health Sciences.

Dr. Neil Batta completed his residency and fellowship training at The Hospital of the University of Pennsylvania, where he excelled as a chief resident, honing his skills in pain management and rehabilitation. His practice interests include advanced pain procedures using x-ray or ultrasound, electrodiagnostic testing, musculoskeletal injuries, and a multidisciplinary approach to recovery. His commitment to patient care and medical education is evident through his current role on the Board of Trustees for the Pierce County Medical Society, where he active works to improve access to healthcare and enhanced healthcare education in local communities. Dr. Batta has been involved in writing a book chapter for medication assisted treatment for opioid use disorder and in original research focusing on blood pressure during fluoroscopic guided injections. Dr. Batta has worked in interventional pain medicine in Washington state since 2021.

Outside of his professional accomplishments, Dr. Batta enjoys spending quality time with his wife, who is originally from Oregon. Together, they relish outdoor adventures, enjoy exploring new destinations through travel, and indulge in great dining experiences at local restaurants. A true Buffalo Bills fan, he never misses a game.

Dr. Neil Batta is passionate about making a positive impact in the lives of his patients and the community combining his expertise in pain medicine and rehabilitation with a personal touch that resonates with those he serves.

NIPUN SHARMA, DO

Dr. Sharma is a board-certified physiatrist with fellowship training in pain medicine, with his practice focusing on diagnosing and treating work-related injuries and motor vehicle injuries.

Dr. Sharma’s treatment philosophy involves using an interdisciplinary approach and maximizing treatment with the least invasive measures possible to provide the best care for his patients. He has pain medicine and interventional spine training and incorporates techniques such as ultrasound-guided peripheral joint injections, intramuscular stimulation, facet joint injections, epidural steroid injections, trigger point injections, radiofrequency ablation, and PRP injections. He is also trained in the use of nerve conduction studies (NCS) and electromyography (EMG).

Dr. Sharma was born and raised in Washington state. He received his bachelor’s in Chemistry at Vanderbilt University, and completed medical school at Des Moines University in Iowa. He completed his residency in physical medicine and rehabilitation (PM&R) at the Rehabilitation Institute of Michigan, and his pain medicine fellowship at Wayne State University/Detroit Medical Center.

Outside of work Dr. Sharma enjoys playing tennis, cooking, reading, baking, rooting for his favorite Seattle sports teams, and spending time with his family. 

ravi Patel, md, pa-c

Dr. Patel is an M.D. and Interventional Pain PA-C with 7 years of experience in primary care, interventional pain management, physiatry and rehabilitation.

Dr. Patel’s love of learning and education drove him to start college studies at age 14, leading to him earning an undergraduate degree in Psychobiology at UCLA, which was followed shortly after by an M.D. degree from Ross University. Seeking an alternative challenge, Dr. Patel then conducted Phase 3 clinical trials at UC Irvine in conjunction with department leads for several years. Following his passion for education and learning, Dr. Patel then taught as a Professor of anatomy and medical sciences before progressing through graduate studies to gain licensure as a board certified PA-C.

Dr. Patel has worked clinically in pain management and rehabilitation for 7+ years with a focus on assessing and managing chronic pain, post-op and post-trauma patients as well as evaluating needs for physiotherapy, neurosurgical intervention, stimulator implants and performing interventional procedures as needed. Dr. Patel has a philosophy of healing which emphasizes conservative therapy and regenerative medicine to achieve optimal patient health outcomes.

Originating from California, Dr. Patel has an enduring love for nature and the great outdoors, exercised through roadtrips, camping, hiking and a good bonfire. Dr. Patel also enjoys comedy, all things vintage and antique, photography, literature, film, and a fine spirit.

RUSSELL “RUSTY” KINDER, MD

Dr. Kinder is a fellowship trained Interventional Pain Specialist and double board-certified in Pain Medicine and Anesthesiology. He has a special interest in advanced interventional techniques.

Dr. Kinder strives to create a positive and mutually rewarding relationship with his co-workers and patients in order to provide the best care possible.  He has a special interest in advanced interventional techniques, and optimizing multidisciplinary support in a personalized, safe, effective, and evidence-based manner.  He believes that when somebody lives with pain, it is important to identify the goals, and then work together with the patient to design a multimodal plan focused on improving overall quality of life and returning to function.

Dr. Kinder is a fellowship trained Interventional Pain Specialist who is double board-certified in Pain Medicine and Anesthesiology.  He was born and raised in Eastern Washington before moving to Seattle to attend the University of Washington, where he completed his Bachelor of Science in Chemistry.  After earning his Doctor of Medicine degree at the Chicago Medical School, he returned to Seattle to complete his medical training at Virginia Mason, where he gained experience at such institutions as Swedish, Seattle Children’s Hospital, and Harborview Medical Center.

Dr. Kinder has presented at several national meetings on topics including medical care efficiency, and management of complications from both anesthesia and interventional pain procedures.  He was first author on the book chapter teaching advanced nerve block techniques in the best-selling textbook, Manual of Clinical Anesthesiology as well as an updated and revised version released in 2020.  He authored a book review of the textbook Acute Pain Management, which was published in the journal Regional Anesthesia and Pain Management (RAPM).  Dr. Kinder was instrumental in reinstating the previously defunct Resident Section of the Washington State Society of Anesthesiologists (WSSA) and served as the President from 2010-11.   He enjoys continuing to keep up with the latest and most advanced pain management options for his patients.

He has spent more than the last decade providing Pain Management in the Pacific Northwest and was the Medical Director of the Interventional Pain Clinic at Adventist Medical Center and member of the Opioid Task Force until early 2020.

In his spare time Dr. Kinder likes to spend quality time with his wife, two daughters and friends.  He enjoys reading, taking in the great outdoors through backpacking, camping, electric skateboarding, watersports, and supporting the Seahawks.  He will “Always Compete” to provide the best care possible

SHELBY DOBBS, PA-C

Shelby grew up in the San Francisco Bay Area and moved to the Seattle area in 1991 when he was stationed at Fort Lewis. He served four years in the Army as a medic, operating in a M.A.S.H. unit before being selected as a flight medic (Air Ambulance). Shelby spent six months in Somalia with a forward-operating medical unit and a year in Korea providing medical support along the DMZ.

After his discharge from the Army at the rank of Sergeant, Shelby attended the University of Washington, where he earned a degree in Zoology. He then pursued his Physician Assistant (PA) studies at Midwestern University in Chicago, graduating in 2002.

Upon completing PA school, Shelby returned to the Pacific Northwest, where he spent 22 years gaining extensive orthopedic experience across all sub-specialties. His areas of interest include joint replacements, sports medicine, and spine care. During this time, he was recognized by his peers as one of the top orthopedic PAs in the Seattle area.

Shelby is passionate about helping patients return to the activities they love. He understands that pain impacts not only physical health but also emotional well-being. His years of experience have equipped him with the skills to integrate various medical disciplines to achieve positive patient outcomes.

Outside of work, Shelby enjoys spending time with his family, exploring the Pacific Northwest, and indulging in his love for food and wine.

Tierra Leonard, LMHC

Tierra Leonard is a Licensed Mental Health Counselor (LMHC) in Washington State, offering virtual counseling services to support individuals in their mental health recovery after a motor vehicle accident (MVA). Her online sessions provide a comfortable and accessible space for patients to engage in therapy from home or the setting of their choice.

Tierra is a Certified Clinical Trauma Professional (CCTP) and a certified Eye Movement Desensitization and Reprocessing (EMDR) clinician who specializes in helping clients find mental and emotional stability after the life-changing impacts of trauma events. Tierra employs evidence-based approaches to treat these symptoms, including Eye Movement Desensitization and Reprocessing (EMDR), Mindfulness-Based Cognitive Therapies (MBCT), Trauma-Focused Cognitive Behavioral Therapy (TF-CBT), psychoeducation, self-compassion techniques, and somatic approaches to help regulate the nervous system.

In addition to treating the experiences associated with car accidents, such as TBI and chronic pain, Tierra also specializes in treating depression, anxiety, grief, and adjustments to each client’s “new normal”. Above all, Tierra is versed in helping clients feel humanized, validated, and accepted exactly as they are at any point in their journeys.

Tierra lives in the beautiful Pacific Northwest with her husband and two amazing sons. She spends her free time enjoying the reprieve of nature, creating art of many different types, and sharing as many moments of laughter and connectedness as she can with those that she loves.
